Addressing the Social Determinants of Health (SDOH)
A defining feature of 2026 PHM is the realization that clinical care only accounts for about 20% of health outcomes. The remaining 80% is driven by Social Determinants of Health (SDOH). Modern PHM systems integrate non-clinical data to address:
Economic Stability: Identifying patients facing food insecurity or housing instability.
Physical Environment: Tracking local air quality or "food deserts" where fresh produce is unavailable.
Health Equity: Segmenting data by race, income, and language to identify and close gaps in care access that lead to higher mortality rates in marginalized groups.
